Community Servings welcomes applications
for our meals service at any time throughout the year. Potential
clients or professionals working with potential clients
are welcome to call our office to request an intake packet
to initiate the intake process. Forms can be mailed or faxed back
to the office.
You will need to complete, sign, date and submit
the following required forms:
- Intake
Form.
- Certification
Form
completed by your primary care physician or nurse. They
will need to fax it back to us (Fax: 617.522.7770)
- If applicable, your PCP needs to call our
Nutritionist if you require a nutritional supplement
- Consent Packet -
Includes the Client Responsibilities,
Rights and Grievance Procedures Form and the Client
Release of Information Consent Form.
- A copy of your Income
Verification. For example: SSI statement, four
pay stubs or a bank statement. (Income verification
is used solely for funding purposes and not for eligibility.)
