Community Servings' home-delivered meals are available to anyone living with a life-threatening illness in our service area, who are unable to shop or cook for themselves. Life-threatening illnesses include, but are not restricted to, HIV/AIDS, cancer, multiple sclerosis, lupus and kidney failure. Eligibility is based on:
- The extent of the illness;
- The client's mobility; and
- Any factors that make it difficult for the client to shop and cook for themselves.
In order to determine eligibility, clients must submit a completed intake packet, including a medical certification form completed by their doctor or nurse. Once paperwork has been submitted, we may contact the client, their doctor or case manager directly for further clarification in order to make the best eligibility assessment.
Our goal is to be there for our clients when they need us the most. Clients can receive our meals for as long as needed and may go on and off our service as medical needs change. Each year clients must be medically certified in order to remain eligible for our meals. If a client is found to be ineligible, s/he may reapply in the future as circumstances may change his/her eligibility status.
